  • If you are a new, incoming student-athlete, please follow the instructions in the link above to register for SportsWare, our injury tracking software. Once we've accepted you into the system, please fill everything out as completely as you can. 
  
•If you are a returning student-athlete, please fill out this paperwork on the computer, print it out, and bring it with you to get your physical.
